In 2002, Dr. Alain Cribier performed the first transcatheter aortic valve replacement (TAVR) in Rouen, France, on a patient deemed too sick for open-heart surgery. Twenty-three years later, TAVR has expanded to treat even low-risk patients. The 2024 PARTNER 3 five-year data showed TAVR produced outcomes equal to surgical valve replacement in low-risk patients — with 3 fewer days in hospital and 6 fewer weeks of recovery.
How TAVR Works
A collapsible bioprosthetic valve is mounted on a balloon catheter, threaded through the femoral artery in the groin, and positioned inside the diseased aortic valve. The balloon expands, pushing the old valve aside and anchoring the new one in place. The entire procedure takes 45–75 minutes. Patients are typically awake under conscious sedation — no general anesthesia, no heart-lung machine, no broken breastbone.
TAVR vs. Open Surgery: The Data
The Edwards SAPIEN 3 and Medtronic CoreValve Evolut are the two dominant platforms. Both have demonstrated non-inferiority to surgical replacement across all risk categories.
Key advantage: hospital stay averages 2–3 days vs. 7–10 for open surgery. Most patients walk within hours and go home within 48 hours.
Key concern: paravalvular leak (blood flowing around the valve rather than through it) occurs in 5–8% of TAVR patients vs. under 1% for surgical valves. Newer-generation valves with sealing skirts are reducing this gap.
International TAVR Pricing
TAVR's biggest barrier is cost — the valve itself costs $30,000–$35,000. In the US, total procedure cost runs $120,000–$180,000. India's Medanta and Fortis Escorts offer TAVR for $25,000–$35,000 using the same Edwards SAPIEN 3 valve. The price difference is almost entirely in hospital and physician fees.
