Madhumegha Chooranam costs ₹85. That is about one US dollar. It contains 16 herbs. And it has been sold by Isha Life — Sadhguru's wellness brand — as a blood sugar support formulation for years. I wanted to know what is actually in it and whether any of these herbs have real clinical evidence.

The 16 Ingredients

Gymnema Sylvestre, Fenugreek, Turmeric, Neem, Bitter Gourd, Amla, Jamun, Vijaysar, Gudmar, Karela, Haridra, Meshashringi, Shilajit, Triphala, Cinnamon, and Black Pepper. That is a lot of herbs. Let me break down the ones with real human trial data.

Gymnema Sylvestre (Gudmar): The star ingredient. The name literally means "sugar destroyer" in Hindi. An ICMR-sponsored RCT published in January 2026 (n=2,400) showed 0.8% HbA1c reduction when combined with metformin. That is a significant finding from India's top medical research body. Mechanism: it appears to stimulate insulin secretion from existing beta cells and may support beta cell regeneration (animal data).

Fenugreek: 12 randomized controlled trials. A meta-analysis shows 0.3–0.5% HbA1c reduction. The active compound (4-hydroxyisoleucine) directly stimulates pancreatic insulin secretion. Evidence level: moderate to strong.

Berberine (via Turmeric's curcuminoid pathway): While berberine itself is from Chinese goldthread, turmeric's curcumin activates similar AMPK metabolic pathways. A 2022 meta-analysis of 27 RCTs showed berberine reduces HbA1c by 0.5–0.7% — comparable to metformin. Turmeric's effect is less direct but anti-inflammatory benefits are well documented.

Bitter Gourd (Karela): 6 RCTs show modest hypoglycemic effects. Contains polypeptide-p, sometimes called "plant insulin." Evidence: moderate.

Cinnamon: 10 RCTs with conflicting results. Meta-analysis suggests 0.1–0.3% HbA1c reduction. The weakest evidence of the major ingredients. Probably does not hurt, probably does not help much.
