Platelet-Rich Plasma (PRP) for Joint Pain: Does It Actual...

Orthopedic Surgery · 2 · August 30, 2025

Kobe Bryant flew to Germany for PRP treatments. Tiger Woods used it after knee surgery. Rafael Nadal's team confirmed PRP use for his chronic foot condition. Celebrity endorsements aside, PRP has been the subject of 47 randomized controlled trials since 2020. The results paint a nuanced picture — not a cure-all, but not snake oil either.

How PRP Works

A nurse draws 30–60ml of your blood, spins it in a centrifuge for 15 minutes, and extracts the platelet-rich layer. This concentrate — containing 5–10x the normal platelet concentration — is injected into the affected joint under ultrasound guidance. Platelets release growth factors (PDGF, TGF-β, VEGF) that theoretically stimulate tissue repair and reduce inflammation. The whole process takes 45 minutes.

What the Best Studies Show

Knee osteoarthritis (grade 2–3): A 2024 JAMA meta-analysis of 22 RCTs (3,400 patients) found PRP provided statistically significant pain relief compared to hyaluronic acid at 12 months — but the clinical difference was modest (10 points on a 100-point pain scale).

Tendinopathy: Strong evidence for lateral epicondylitis (tennis elbow). A 2024 BJSM study showed 78% improvement vs. 52% for corticosteroid injection at 12 months.

Rotator cuff tears: Mixed evidence. PRP may enhance healing of small partial tears but doesn't replace surgery for full-thickness tears.

The Cost Calculation

PRP costs $500–$2,000 per injection in the US, $200–$500 in India, and $300–$600 in Turkey. Most patients need 2–3 injections spaced 3–4 weeks apart. Insurance rarely covers it. For knee OA, compare the total PRP cost ($1,500–$6,000 for a series) against a knee replacement ($6,800 in India) that provides decades of relief.

Key Takeaways

- PRP provides modest but real pain relief for knee osteoarthritis (grade 2–3) — don't expect cartilage regeneration.

- Strongest evidence is for tennis elbow: 78% improvement vs. 52% for corticosteroid at 12 months.

- At $500–$2,000 per injection in the US, PRP may not be cost-effective compared to joint replacement abroad at $6,800.
