The left internal mammary artery (LIMA) graft to the left anterior descending (LAD) artery is the single most durable intervention in cardiac surgery — 95% patency at 15 years. Saphenous vein grafts to other arteries? Only 50% remain open at 10 years. Hybrid coronary revascularization combines the best of both: LIMA-to-LAD bypass through a small thoracotomy, then stenting of the remaining vessels in the cath lab.

The Logic Behind Hybrid

The LAD supplies 40–50% of the left ventricle's blood. A patent LIMA-to-LAD graft is the strongest predictor of long-term survival after coronary revascularization. Drug-eluting stents work well for non-LAD vessels — their 5-year patency (92–95%) exceeds saphenous vein grafts (75–85%).

Traditional CABG uses vein grafts for all non-LAD targets. Hybrid uses the LIMA for the LAD (surgically, through a small incision) and DES for everything else (percutaneously, through the wrist or groin). The patient gets the best conduit for the most important artery and the least invasive approach for the rest.

Clinical Results

A 2024 JACC meta-analysis of 3,200 hybrid patients vs. matched traditional CABG patients found: identical 5-year survival (93% vs. 92%), identical MACE rates (major adverse cardiac events), but 50% shorter hospital stay (3 days vs. 6) and 60% faster return to work (2 weeks vs. 5 weeks). The approach works best for patients with 2–3 vessel disease where the LAD has a critical proximal lesion.
